Medical Clinic Startup Costs: A Practical Planning Checklist

Understand the one-time expenses, monthly obligations, and working-capital needs that belong in an early clinic budget.

Why a single number can be misleading

Clinic costs vary with the market, square footage, construction, services, equipment, staffing, licensing, and opening schedule. A preliminary range can start a conversation, but it is not a substitute for local quotes and a full operating model. Primary care and same-day clinics may cost less than urgent care facilities with extensive buildout and equipment.

One-time startup expenses

Typical categories include legal and professional work, deposits, design and construction, furniture, medical equipment, computers, phones, records and practice-management systems, licenses, insurance deposits, signage, initial supplies, recruiting, training, and opening marketing. Each category should have an owner, estimate, source, and contingency.

Monthly operating expenses

Rent, payroll, benefits, clinical coverage, software, billing support, insurance, utilities, supplies, waste services, maintenance, professional services, and marketing continue after opening. The SBA recommends separating one-time and monthly costs to create a clearer capital picture.

Working capital is not optional

A clinic may spend money before credentialing, claims, and collections produce steady cash flow. Build a monthly cash-flow plan with conservative assumptions and a reserve for delays. Revenue and break-even timing should be scenarios, not promises.
